By using the Bausch + Lomb Access coupon, you confirm that you understand and agree to comply with the following terms and conditions of this offer:
Walgreens is a well-known and respected retailer with more than 8,200 locations in the United States. The alignment will allow patients access to Bausch + Lomb products at a consistent, affordable price. This may help to minimize pharmacy callbacks.
Ninety percent of our scripts are covered within the Walgreens network. We are looking at areas where coverage is low or absent to align with independent pharmacies. PROLENSA® and LOTEMAX® GEL Part D patients can go to any pharmacy.
You will be provided with a map and list of locations by zip code. Please become familiar with Walgreens in your area so you can provide information to your patients.
Yes, for commercial and cash patients, these cards would replace the current programs. Part D for LOTEMAX® GEL and PROLENSA® will continue with current programs and coupons and will expire December 31, 2016.
The current coupons will continue to work at all pharmacies until April 30, 2016. On May 1, 2016, all commercial and cash patients must go to Walgreens for the discount program.
A patient can use the card up to 6 times. If more than 6 fills are needed, another card is required.
Just like our current programs, patients are required to have a coupon, so make sure to emphasize this to patients. Coupons will be available at Walgreens and online at Bausch.com/accessprogram.
Most eligible insured commercial patients will pay no more than $35. Most eligible uninsured patients will pay no more than $60.
